This was sent to me by a friend, its legit... following is the story from him... __________________________________________________ Okay guys here is the info on this voicemail. One of my friend's from work and her friend were out one night in the SF Marina district and were hanging outside of the bars trying to find a cab. One of the girl's, Olga ends up meeting this guy Dmitri and they talk for at the most 2 minutes. She hands him her business card and says call me. Well attached is the actual voicemail that this guy left her. Wait till you hear it you will be laughing so hard you'll fall out of your chair.

The dying man's wish was to sterilize his retarded daughter. The old farmer was terminally ill and his dying wish was to have his mentally retarded 18-year-old daughter sterilized. His wife, the mother, strongly agreed and both met with me regarding this delicate topic. Their daughter was to eventually become a guardian of the state of California. The parents feared that their daughter might get raped in an institution or somehow become pregnant with a child she could never care for. The grandchild would end up under the care of social services or foster care. The old farmer and his wife were asking me to do the impossible. Though I understood their dilemma, I told them that public laws made the requested tubal ligation impossible. I suspect, however, that the financially well off arrange for such procedure through private surgical clinics in places like Beverly Hills. The old farmer and his wife did not have this option. There are many ethical dilemmas in medicine that must be decided by the immediate family and the physicians involved. In this video, the twins are mentally retarded and the mother is in constant fear that they may be sexually abused. If the little girls have any vaginal itch or discharge, both common, the mother cannot be appeased until I examine them for any sexual abuse. The mother works two jobs and refuses to ask for social security on behalf of her twins, though they qualify. These twins will never function independently. Were she to die, the mother worries deeply as to what will become of her daughters. The father is detached and uninvolved with his kids. Doctors must lobby society in order to restore a more ethical approach on behalf of similar parents. Up to three percent of children born in the United States are mentally retarded. We need some public policy that will guide desperate parents and physicians in regard to those that are severely retarded and profoundly impaired. The role of religion or society at large in these private matters may only complicate the issue and may impose guilt upon desperate parents. In the video you will see a lady with a big bulge over the epigastric area, just below the breasts. This lady had umbilical hernia surgery 5 years prior to presentation. The bulge began to grow after giving vaginal birth to her second child two years ago. This lady is being evaluated at our county hospital and an update is to follow. External otitis is commonly associated with anxiety and/or depression. Before I approach the topic of anxiety or depression with such patients, I first gently clean the ear and reduce the associated pain with topical drops. I first make human contact. I may then say to the patient that an itchy ear may be compounded by stress. That's when I began my psychotherapy. Another case in the video is of a very toxic child who was febrile and sleepy at first. These can be ominous signs. His neck was supple (a reliable sign after 18 months of age) and bacterial meningitis seemed unlikely. He had allergy related disease that included sinusitis, otitis media and bronchial asthma. His lack of oral intake made him feel sicker because of depleted glycogen stores and ketone production. Two hours after initiating treatment that included Ceftriaxone 500 mg IM, Dexamethasone Phosphate 4 mg IM and oral fluids, he was much better. He recovered to his usual active self. The patient with the sunken chest (pectus excavatum) did not require the surgery that had been recommended. The leg hemangioma (Strawberry nevus) resolved with time. No surgery was needed. The child with seborrheic dermatitis that affected his scalp and skin (behind the ears) was treated with Selsun Blue Shampoo, three times per week, hydrocortisone cream 1% and Cephalexin for his secondary bacterial infection (impetigous). Selsun Blue and hydrocortisone cream are available over the counter. I did not use an anti-fungal cream, i.e. Clotrimazole. The 15-year-old boy with a mass over his jaw was due to an abscess that originated over his left lower molars. I initiated treatment and referred him to a dentist. The venous stasis ulcer may take up to two months to heal with the proper care. Clinicians must avoid closing an infected wound. Facial wounds, however, may be an exception to certain wound closure rules. I have seen serious complications when dog bites are sutured (closed). I have not used Betadine for wound care or soaks for over 15 years since it may be toxic to the normal tissue that is needed to heal the wound. Wound debridement and irrigation with copious amounts of saline water can be very useful. Clinicians must never forget to update patients for Tetanus/Diphtheria (Td) when appropriate. The tetanus vaccine should be given, whenever possible, within 24 hours from injury. Luis Lomeli MD/Beta
