This tutorial provides information on how to file FCC Form 498 (Service Provider Identification Number and Contact Information Form) to service providers participating in the federal Universal Service Fund using USAC's E-File System. Form 498 is used by service providers to provide contact and remittance information for USF payments for each of the programs they participate in (High Cost, Low Income, Rural Health Care, Schools & Libraries).
