In the video, you will see two disastrous medical cases. One child had been healthy until age 1 year when she developed a fever associated with a convulsion. The mother took her to a doctor who treated only the fever with an injectable form of Neo-Melubrina (Dipyrone is the active ingredient) and no antibiotic. Dipyrone is an unsafe drug that can cause agranulocytosis (lack of white blood cells) and has been banned in many countries including the United States. A patient with meningitis may present with a febrile seizure and clinicians must always consider this diagnostic possibility. The child is now severely impaired. The clinical image reveals decorticate rigidity, Posturing, that consists of flexion of the elbows and wrists and supination of the arm. Meningitis is a medical emergency and I institute medical therapy immediately. The combination of Ceftriaxone (Rocephin ) and Dexamethasone IM has saved all of my children patients. Though Ibuprofen (Advil ; Motrin ) is widely prescribed to treat fever in children, I do not recommend it. At times doctors tell parents to dose a child with both Ibuprofen and Acetaminophen. I simply optimize the Acetaminophen (Tylenol s active ingredient) dose and I recommend cooling measures. For highly febrile patients, dexamethasone, a corticosteroid (non-salt retaining) can help reduce the fever if unsafely elevated. Ibuprofen retains salt and may increase the risk of a convulsion. I have seen serious reactions due to Ibuprofen. How about taking Ibuprofen (Advil ; Motrin ) or Naproxen (Aleve ) for a headache? A patient may not know that a headache may be due to a brain hemorrhage, subarachnoid bleed, and these drugs may precipitate death. These drugs, like Bayer s aspirin, promote bleeding. In the event of a traumatic auto accident, a few minutes can be the difference between life and death. I always consider Murphy s Law, if it can happen, it will. What I recommend for my patients with tension headaches is a combination of Diphenhydramine (a sedating over-the-counter anti-histamine that saves lives when patients react to an immunization or drug) and Acetaminophen (active ingredient in Tylenol ). Tylenol PM consists of DIPHENHYDRAMINE (which is also contained in Benadryl Allergy ; Sominex ) and ACETAMINOPHEN. You may choose to pay over twice the cost by buying brand named products; I don t whenever possible. If all that you need is sedation, why take Tylenol PM when all you need is the Diphenhydramine ingredient as in Sominex . Your pharmacist can assist you if you have any questions. How about Advil PM caplets? They contain Ibuprofen 200 mg and Diphenhydramine 38 mg. Does it sound familiar? Advil PM is another run-of-the-mill product that you can buy individually as generics for less than half the cost. For old age osteoarthritis, I recommend acetaminophen since there is little inflammation. Advil (Ibuprofen) however is marketed as a drug that targets the pain site. In fact, Advil targets all of your cells and may cause you gastrointestinal bleeding. If you do need pain control and sedation buy a generic substitute for Tylenol PM (ask your pharmacist). Drug companies are very good at marketing plain aspirin-like drugs as real PAIN KILLERS as was the case with Vioxx . One patient I once treated took a bunch of his mother s Vioxx pills and waited for a high that never came. Instead, he came to see me for Vioxx related side effects. Your doctor should prescribe responsibly and with intelligence. After all, the public (tax payers) paid for his medical education and now pays him to treat you. Without a third party to pay doctors (employer or public), our health care system would collapse. Let us protect the healthcare dollar. You should not take non-sedating antihistamines like those contained in Allegra-D or Clarinex-D . These also contain a high dose of PSEUDOEPHEDRINE (drug used to make crystal Meth), which often causes insomnia. Consumers must learn the chemical PSEUDOEPHEDRINE. It should be avoided by those with a hyperactive thyroid, at risk for a brain bleed/hemorrhage, hypertension, heart disease and other medical conditions. The child in the video with scissor walking was injured at the worst hospital I have ever seen in the United States. The infant received an air embolus via the intravenous line and suffered a cardiac arrest. The child is now severely impaired. This was a malpractice case that was settled for about 2 million dollars. The best person to protect you in our complex healthcare system can be a registered nurse (R.N.).
